Patient Details

Ashbourne Medical Practice offers a number of services which require your contact details, we may also need to contact you urgently regarding your own, or a relatives, healthcare. We therefore ask you to ensure you provide us with your most up-to-date contact details.

Change of Address.

When changing your address, please take a moment to look at our practice boundary map, found under the practice area section, to check your new address is in the area that we cover.

If you live outside of this boundary area, we may still be able to register you as an 'out of area patient'. This would entitle you to receive all of our usual services, with the exception of home visits.

Please ask at reception for more details.    

Text Confirmations and GP Calls

When you book an appointment you can request a text confirmation and reminder. This helps you remember your appointment and reduces the number of wasted GP appointments. The Doctors also require your contact number to gain and pass on information to you regarding your care. We therefore recommend you provide us with your existing mobile and home phone numbers. 

Change of Name

If you need to change your name with us, you will need to provide proof of this.
