Once registered with the practice you can sign up for online access to order repeat prescriptions and to book GP appointments.
To enable us to do this, we will need to see a form of photographic identification.

Summary Care Record is an electronic record that will give healthcare staff faster, easier access to essential information about you, to provide you with safe treatment when you need care in an emergency or when your GP practice is closed.
If you wish to opt out of this service you need to make reception aware on registering with the practice.

Ashbourne Medical Practice offers a number of services which require your contact details, we may also need to contact you urgently regarding your own, or a relatives, healthcare. We therefore ask you to ensure you provide us with your most up-to-date contact details.
When changing your address, please take a moment to look at our practice boundary map, found under the practice area section, to check your new address is in the area that we cover.

When you book an appointment you can request a text confirmation and reminder. This helps you remember your appointment and reduces the number of wasted GP appointments. The Doctors also require your contact number to gain and pass on information to you regarding your care. We therefore recommend you provide us with your existing mobile and home phone numbers.
If you need to change your name with us, you will need to provide proof of this.
If you are ill whilst on holiday or working away from home you can receive treatment from the local GP practice for up to 14 days as a Temporary Resident. If you are not registered with a Doctor but need to see one, you can receive emergency treatment from the local GP practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a Temporary Patient although they do have an obligation to offer 'urgent' treatment. You cannot register as a Temporary Patient at a practice in the town or area where you are already registered.

It is possible in some circumstances to be registered as a Temporary Patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
